Child Consent Form

Unless children are accompanied by both parents, any child under the age of majority that is travelling internationally should have a valid passport and visa (if required) and may need a notarized letter from one or both parents granting permission for their child to travel. This applies to children travelling with one parent, grandparents or other family members or friends, or travel for sports or school trips. Legal guardians travelling with a child should travel with notarized proof of guardianship. Please note that the link below redirects to a Government of Canada webpage that includes an interactive form to write a consent letter. It also includes general recommendations about travelling across borders with children. If the child is not Canadian, we recommend you consult with local authorities to determine if there are any requirements above and beyond the form in the link below. Carrying a consent letter does not guarantee that children will be allowed to enter or leave a country, as every country has its own entry and exit requirements. We also recommend that you contact the airline, bus, train or other transport company the child will be using to check its policies and regulations for child travelers.

By sending applications to CIBT for processing, you accept all of the requirements, restrictions and limitations on liability set forth herein and specifically agree and confirm that neither CIBT nor your travel company shall be liable to you for any failure to comply with the necessary visa requirements for your travel, for the action or inaction of any government body and/or for the performance by any third party delivery company.

At all times and under all circumstances, CIBT's total liability to you for any and all claims, causes of action, liabilities and damages of any kind shall not, in any circumstance or for any reason, exceed the lesser of the fees actually paid by you to CIBT or $500, and must be submitted to CIBT within 90 days of occurrence. In no event shall CIBT be liable to you for any indirect, special, consequential or incidental losses or damages, including, without limitation, lost profits, or for punitive damages, and that your sole and exclusive remedy against CIBT for damages in connection with the failure to obtain a requested visa or visas in a timely manner shall be the return of the CIBT service fees actually paid by you or $500, whichever is less. When CIBT returns your passport and documents, it is important that you verify that all the visas you require for your trip have been obtained, that the visas for each

CIBT will pay $20 commission per visa or passport (note this applies for paper visas only – electronic visas are not eligible for commissions). Commissions are paid on a quarterly basis. Travel Edge also has a 10% discount over retail rates when working with CIBTvisas.

A report detailing the visa and passport transactions will also be included with the commission check.
